How to setup OTP Verification for Form Maker Form?
Form Maker is a contact form builder plugin. Form Maker is used to create forms for any purpose, like simple contact form to multi page survey forms. OTP Verification Plugin verifies Mobile Number of users by sending OTP Verification code before submitting Form Maker Form. The purpose of this guide is to help you setup OTP Verification for your Form Maker Form.

Follow these steps to setup OTP Verification on Form Maker Form
1. Setup Phone Verification for Form Maker Form
- Go to the Forms tab in the OTP Verification plugin then search and select Form Maker Form from the list.
- Enable checkbox next to Form Maker Form.
- Enable Phone verification and click on Click here to see your list of forms.
- Select your Form from the list and note the Form ID.
- Click on Edit link of your form to add new field.
- Add new Phone field if it doesn't already exist and make sure to mark the field as required.
- Note the field label of phone. Click on the Update button to save the added field.
- Add a single line text field where users will enter the verification code and Make sure to mark the field as required
- Note the field labels of verification code. Click on the Update button to save the added field.
- After that click on the Update button for save your form.
- Now go to the Forms Settings page and enter the Form ID, Phone field Label and Verification field Label.
- After that click on the Save Settings button.
- Go to your Form Maker form page, fill all the required field and click on Click Here to send OTP.
- Enter the OTP received in the Verification Field and click on the Submit button.
- Form will be submitted after successfully OTP verification.
2. Setup Email Verification for Form Maker Form
- Go to the Forms tab in the OTP Verification plugin then search and select Form Maker Form from the list.
- Enable checkbox next to Form Maker Form.
- Enable Email verification and click on Click here to see your list of forms.
- Redirect to the Form Maker Forms page.
- Select your Form Maker Form template from the list and note the Form ID.
- Then click on the Edit link.
- Add new Email field if it doesn't already exist and make sure to mark the field as required.
- Note the field label of email. Click on the Update button to save the added field.
- Add a single line text field where users will enter the verification code.
- Note the Field labels of verification code. Click on the Update button to save the added field.
- After that click on the Update button for save your form.
- Now go to the Forms Settings page and enter the Form ID, Phone field Label and Verification field Label.
- After that click on the Save Settings button.
- Go to your Form Maker form page, fill all the required field and click on Click Here to send OTP button.
- Enter the OTP received in the Verification Field and click on the Submit button.
- Form will be submitted after successfully OTP verification.
3. Change verification button text
- Go to the Form settings page.
- Enter the text you want to use for the verification button and click on the Save Settings button.
